Capstone Project form
Laboratory Safety Contract
- All physics laboratory students must complete this form prior to participating in laboratory activities. This includes but is not limited to all students in PHYS 102, 116, 216, 209A, 209B, 313L, 366 and all students doing capstone projects or research in laboratory spaces.
Registration and Records Forms
- Change of major, Graduation Application, Petition to Take Extra Units, Time conflict and more.

- You will need to include the following participants on the form. Participant 2 the PI/Instructor you are working with, participant 3 Monica Lares ([email protected]) and participant 4 Tai Russotti ([email protected]).
