  • All physics laboratory students must complete this form prior to participating in laboratory activities. This includes but is not limited to all students in PHYS  102, 116, 216, 209A, 209B, 313L, 366 and all students doing capstone projects or research in laboratory spaces.

  • Change of major, Graduation Application, Petition to Take Extra Units, Time conflict and more.
• Student Key Request
  • You will need to include the following participants on the form. Participant 2 the PI/Instructor you are working with, participant 3  Thomas Targett (targett@sonoma.edu) and participant 4 Julie Barnes (barnesj@sonoma.edu).  Review the step-by-step instructions on how to complete the form and add the participants.
